What to Bring on Your Initial Evaluation Visit

On the day of your appointment, please bring the following:

  • Insurance card(s)
  • Drug prescription card(s)
  • Photo Identification
  • Other pertinent authorization numbers, forms or referrals
  • Current list of medications that you are taking
  • Current list of all your doctors and their telephone numbers

Tips for Talking with your Healthcare Team

  • Prepare your questions in advance
  • Bring a friend or relative to the appointment to help you remember the information
  • Take notes
  • Speak up if you don't understand something
  • Ask for a language interpreter if needed

During Your Initial Evaluation Visit

Your initial evaluation consists of the following:

  • Blood tests (you may eat breakfast)
  • Transplant information session
  • Meeting with your Transplant Clinical Coordinator
  • A history and physical exam (performed by a Transplant Physician)
  • Meeting with a Transplant Surgeon (meeting will occur during this visit or at a later date (depending upon availability)
  • Interview with a Social Worker and a Financial Coordinator

This visit will take approximately five or six hours; if you cannot stay for the entire initial visit, we encourage you to reschedule your appointment or you will need to schedule a second visit to complete the initial evaluation.

You may bring something to eat or you can purchase food in the immediate neighborhood.
If you are a CAPD (peritoneal dialysis) patient and will need to do an exchange, please bring your solution and supplies.

For Recipients with Potential Donors Only:

If you have a potential donor(s), the donor(s) should plan to do the following:

  • Call 212-746-3922 to register BEFORE the initial appointment.
  • Attend the Transplantation Information Session.
  • Have blood drawn during the initial appointment.
